a) postal address of the residential (non-residential) premises, information about the owner (owners) of the premises (indicating the name of the legal entity or surname, first name and patronymic of an individual), and for residential premises of state and municipal housing funds - information about the tenant of the residential premises (with indicating the last name, first name and patronymic of the employer);

b) the name of the contractor (indicating the name of the legal entity or the last name, first name and patronymic of an individual entrepreneur), his bank account number and bank details, address (location), contact telephone numbers, fax numbers and (if available) email addresses, address performer's website on the Internet;

c) an indication of the paid month, the name of each type of paid utility service, the size of tariffs (prices) for each type of corresponding utility resource, units of measurement of volumes (quantities) of utility resources (when using hot water tariffs in payments for utility services for hot water supply, consisting of a component for cold water used to provide public services for hot water supply, and a component for thermal energy used to heat water for the purpose of providing public services for hot water supply - the value of each component, units of measurement of the volume (quantity) of hot water and thermal energy in natural quantities);

d) the volume of each type of utility services provided to the consumer during the billing period in residential (non-residential) premises, and the amount of payment for each type of utility services provided, determined in accordance with these Rules;

d(1)) the amount of the increasing factor provided for in paragraph 42 of these Rules, in the case of applying such an increasing factor when calculating the payment for the relevant utility service, as well as the amount of excess of the payment for the relevant utility service, calculated using the increasing coefficient over the amount of payment for such utility a service calculated without taking into account the multiplying factor;

e) the volume of each type of utilities provided during the billing period for general house needs for the cases provided for by these Rules, per each consumer, and the amount of payment for each type of such utilities, determined in accordance with these Rules;

f) the total volume of each type of utilities for general house needs provided in an apartment building in the cases provided for by these Rules for the billing period, readings of the collective (community) metering device for the corresponding type of communal resource, the total volume of each type of utilities provided in all residential and non-residential premises in an apartment building, the volume of each type of communal resource used by the contractor during the billing period in the production of utility services for heating and (or) hot water supply (in the absence of centralized heating and (or) hot water supply);

g) information on the amount of recalculation (additional charge or reduction) of fees for utility services, indicating the grounds, including in connection with:

→ use of residential premises by temporary residents;

→ provision of utility services of inadequate quality and (or) with interruptions exceeding the established duration;

→ temporary absence of the consumer in the occupied residential premises, not equipped with individual and (or) common (apartment) metering devices;

→ payment by the contractor to the consumer of penalties (fines, penalties) established by federal laws, the agreement containing provisions for the provision of utility services, and these Rules;

→ other grounds established in these Rules;

h) information on the amount of consumer debt to the contractor for previous billing periods;

i) information on the provision of subsidies and benefits for the payment of utility services in the form of discounts (before the transition to the provision of subsidies and compensation or other measures of social support for citizens in cash);

j) information about installment plans and (or) deferment of payment for utility services provided to the consumer in accordance with paragraphs 72 and 75 of these Rules;

k(1)) bar codes provided for by GOST R 56042-2014 (when the owners of premises in an apartment building managed by a homeowners association or a housing cooperative or other specialized consumer cooperative, as well as when directly managing an apartment building, decide to refuse to indicate in the payment document bar codes provided for by GOST R 56042-2014, such bar codes are not indicated in the payment document);

k) other information to be included in payment documents in accordance with these Rules, regulations governing the procedure for establishing and applying social norms for the consumption of electrical energy (power), and the agreement containing provisions for the provision of utility services.

In a payment document issued to a consumer of utilities in an apartment building, in the case established by the second paragraph of clause 40 of the Rules, the payment for utility services for general house needs and the payment for utility services provided to the consumer in residential or non-residential premises must be indicated on separate lines .

Utility providers

The CG provider is the organization that provides them to consumers.

In an apartment building, the executor can be a management organization, a partnership, a cooperative, or a resource supply organization - a supplier of a certain resource.

If there is no metering device, the amount of the fee is determined according to the standards and is calculated using a different formula: the number of people living in the apartment or house is multiplied by the consumption standard and the price of the utilities.

If it is technically possible to install the device, but there is no metering device, the calculation formula has a multiplying factor of 1.5.
