In accordance with the current legislation, the place of residence of any citizen is considered to be a certain housing in which a person resides on a permanent or temporary basis, and in the event of a change of residence address, one must first deregister at the previous one, and only then register at the new one.

To carry out this procedure, you need to contact the government agency responsible for processing and issuing the relevant documents by submitting an application. After completing all the documentation, the citizen is issued a passport, which contains a note about the discharge, as well as a certificate of departure from the previous place of residence.

In this case, there are two options for carrying out this operation - submitting an appropriate application when moving or after arriving at a new address, where you can issue an extract along with your registration and, thus, be deregistered completely automatically.

In the first case, it is enough to contact the government agency located at your previous place of residence and submit an application along with your passport, then wait for the departure certificate to be issued and receive it in your hands.

The second option has a lot of advantages, since a person does not have to come to the passport office twice to register, and it is quite enough to simply visit this authority at the new place of residence and contact the appropriate authority, after which you will immediately receive both a departure certificate and documents confirming the registration of the new registration.

    When deregistering on your own, you must write an application (sample) and fill out Form 7 in two identical copies. How to correctly fill out the Form 7 departure address sheet?

    Important! The application and Form 7 are filled out only on the basis of documents (general passport, birth certificate for minors, etc.) of the person who is being discharged from this living space.

    There are 17 points on the departure sheet, of which 1 to 12 are required, which reflect the following information without abbreviations or errors:

    1. Name.
    2. Surname.
    3. Middle name (if available).
    4. Date of birth in the format “DD, MM, YYYY”: the month is written in words, the day and year – in numbers.

    6. Place of Birth.

    Filling process

    Information is entered on a computer and the completed sheet is then printed. The citizen’s basic details are indicated in full accordance with the data contained in his passport.

    Rules and instructions:

    1. Blots and marks on both sides of the form are prohibited;
    2. When indicating the date of birth, the name of the month is entered in words, the day and year - in numbers. To indicate the gender, the person filling out the form should highlight the appropriate option (male/female) with one horizontal line using a blue or black ballpoint (gel) pen;
    3. Section 12, which in its main part duplicates Section 8, is required to be completed. Paragraphs 13-15 are prescribed if necessary: ​​when a citizen moves to a new place without changing the locality, as well as changing his full name, gender, date of birth. Line 15 briefly describes the reasons for adjusting the basic details;
    4. In paragraphs 16 and 17, the recipient of the sheet and the employee of the passport office receiving it put a personal signature by hand and note the date of filling out the form. The machine-readable data block is filled in by the registration authority employee after the recipient of the certificate is issued.

    Although the departure address sheet is not a mandatory document for registration at a new place of residence, it is still better to issue it, because, as practice shows, upon new registration it may still be required by a house management employee. Another advantage of issuing this certificate is that it is free and easy.

    Filling rules

    Points 1–11 are located on the front side. Information must be entered into them
